Nasolabial folds

A fold is almost never treated where you see it. The cause usually sits higher — in lost midface volume — and that is what has to be addressed, or you end up with a heavier face rather than a smoother one

What it is

A fold is not a wrinkle. It is a shadow

Everyone has nasolabial folds: they are the anatomical border between the cheek and the area around the mouth

With age they deepen not because the skin has creased, but because the midface loses volume and soft tissue drifts downwards. The fold deepens as the shadow of that shift. Which is why it is usually corrected not in the groove itself but higher up, by restoring support in the cheekbone area.

Step by step

What to expect

01

Reading the whole face

The doctor looks at proportions rather than the fold:

where volume has been lost, how tissue behaves at rest and when you smile. That decides where the product goes — and whether it belongs in the fold at all

02

Restoring support

Most often the product goes into the midface:

restored volume lifts the tissue and the fold softens on its own. Placement into the groove itself is done carefully and not always — with a descended midface it creates a ridge

03

Assessment at two weeks

Swelling in the first days is normal

The final result is judged at two weeks, once the product has settled; that is also when any touch-up is decided

Common questions

The essentials, answered

Can nasolabial folds be removed completely?

No — and nobody should promise that: everyone has these folds, they are an anatomical border. The goal of treatment is different — to restore volume where it has been lost so the groove stops reading as a sign of age. The difference between softening and erasing matters here.

Is filler injected straight into the fold?

Not always, and most often no. With a descended midface the product goes higher, into the cheekbone area: restored support lifts the tissue and the fold softens by itself. Placing filler into the groove when tissue is already sagging creates a ridge and weighs the face down. Where it goes is decided after an examination.

How long does the result last?

Usually 9–12 months, depending on the product, the amount of correction and your metabolism. You see a first result immediately; the final one comes at two weeks, once swelling has gone and the product has settled.

What if I do not like the result?

Hyaluronic acid fillers are reversible — the product is dissolved with an enzyme (filler removal from ₽7,500). Any decision about correction is made no earlier than two weeks, when you are looking at the real result rather than at swelling.

Do creams and facial massage help?

Skincare supports skin quality, which is worth doing, but it does not restore midface volume — and that is what deepens the fold. Massage and facial exercises work on muscle tone and are likewise no substitute for volume. Expecting the fold to disappear from either is unrealistic.

Why “just fill the fold” is a bad idea

If the midface has descended and filler goes into the groove itself, tissue gains extra weight exactly where it is already sagging. The fold flattens for a while, but the face grows heavier — and within months the groove returns deeper than before. Your doctor will decline to do it and explain why.
