I, a user of the website re-face.ru (the “Website”), acting freely, of my own will and in my own interest, by ticking the consent box in a form on the Website and submitting it, give Lazer Limited Liability Company (RE/FACE clinic; INN 2700033608, OGRN 1242700010321; address: 49 Turgeneva St, premises I (22–26, 135–140), Khabarovsk, Khabarovsk Krai, 680000, Russia) — the “Operator” — my consent to the processing of my personal data on the terms set out below.
What data
- my name — as I entered it in the form;
- my phone number, email address, Telegram or MAX handle — whichever of these I provided myself;
- my preferred contact method;
- the answers I voluntarily gave in the questionnaire (quiz): age range, skin type, aesthetic goals and concerns about skin or hair, treatment experience;
- the Website page and form the enquiry was sent from, and its date and time.
For what purposes
- contacting me about my enquiry — a call back or a message in the messenger I selected;
- booking a consultation or appointment at the clinic;
- preparing personal recommendations from my questionnaire answers;
- keeping a history of enquiries;
- considering my job application, if I submitted one through a form on the Site.
This consent does not cover promotional or informational mailings — if I wish to receive them, I will give a separate consent.
What operations
Collection, recording, systematisation, accumulation, storage, updating, retrieval, use, transfer (provision, access) to the persons listed in section 04, anonymisation, blocking, deletion and destruction — by mixed means, with and without automation.
Who may process the data on the Operator’s behalf
- amoCRM — for enquiry management and the history of communication with me;
- YClients — for online booking and the clinic schedule;
- Callibri — to identify which page of the Site my call came from and to log the enquiry in the shared history.
Transfer is made under contracts, subject to confidentiality and personal data protection requirements. The databases are located in the Russian Federation.
Duration and withdrawal
This consent is valid until the purposes of processing are achieved or until it is withdrawn. I may withdraw it at any time by sending the Operator a notice to refaceclinickhv@gmail.com marked “Withdrawal of consent to personal data processing”, or in writing to the Operator’s address.
On receiving the withdrawal, the Operator ceases processing and destroys my personal data within 30 days, except where the law allows processing without consent.
Confirmations
- I am 18 or older, or I act with the consent of my legal representative;
- the data I provided is accurate and relates to me;
- I have read the Operator’s Personal Data Processing Policy.
