RF microneedling: what happens in the skin and why a course is needed

One of the few methods that neither fills nor relaxes, but makes the skin build its own collagen. We look at how microneedling RF differs from conventional RF, which problems it solves and why the result does not arrive on the day of the treatment

Most aesthetic treatments do one of two things: they add something to the skin or they dial something down in it. A filler adds volume, botulinum toxin relaxes a muscle, a hydrating product replaces what is missing. RF microneedling is built differently: it adds nothing, it creates the conditions in which the skin starts building new collagen itself.

Everything else follows from that — why the result is not visible as you leave the room, why a course is needed, and why the effect lasts a long time. Let us take it in order.

What physically happens in the skin

The handpiece with its cartridge sets a grid of micro-punctures in the skin. The needles pass through the epidermis and deliver radiofrequency energy directly into the dermis — the layer where fibroblasts live and where the collagen framework sits. The surface takes almost no part in the process: it is a door, not a target.

At the chosen depth the tissue is heated to 52–55 °C. At this temperature two things happen at once. Existing collagen fibers contract — that is the tightening visible within the first few days. And remodeling begins: fibroblasts read the controlled heating as a signal to repair and start synthesizing new collagen. The epidermis meanwhile is heated only to 40–42 °C, so the skin stays intact.

How this differs from conventional RF and from microneedling

It is a fair question: microneedles and radiofrequency have each existed separately for many years. The difference is in how far the action reaches.

Microneedling without energy works mechanically and stays in the upper layers. Radiofrequency without needles has to travel through the epidermis, which limits its power: heating the surface too strongly is unsafe. Combining the two removes that limit — the energy reaches exactly where it is needed, in the dose the doctor has set.

On SALANO Optimus this is governed by several parameters at once: insertion depth from 0.4 to 4 mm, a cartridge with 18, 25, 49 or 81 needles, needle type (insulated ones heat only the tip, non-insulated the full length), and monopolar or bipolar mode. The skin around the eyes and the skin on the body call for fundamentally different settings — which is why the treatment is performed by a doctor following a protocol for the area, not by launching a general program.

Which problems the method solves

The indications are fairly broad, but they share one thing: all of them relate in some way to the state of the collagen framework and the quality of the dermis.

Phototype deserves a separate word. Light-based methods — IPL, lasers — work through the absorption of energy by pigment, and on darker skin that carries a risk. Radiofrequency is not absorbed by melanin, so RF is equally safe on any phototype. For some patients it is the only device-based way to work on skin density.

How the treatment goes and what follows

It starts with cleansing and a topical anesthetic, applied in advance so it has time to take effect. The doctor then works through the area with the handpiece, changing the cartridge and the settings while moving from one zone to another. Afterwards the area is cooled with the CryoCool attachment and a repair formula is applied. How long it takes depends on the extent of the work and the method — your doctor will give you an exact time after the examination.

Recovery after a microneedling treatment is short, but it exists. Redness and swelling last 3–5 hours, erythema may persist for 1–2 days, and sometimes fine crusts appear — they clear within 3–7 days. There is no need to be alarmed: this is what a repair process under way looks like. After non-invasive fractional RF there are practically no restrictions and you can go straight back to your day.

Why the result builds over months

Collagen synthesis is not a fast biological process. Fibroblasts do not produce new fibers overnight: they need time to build the structure, and the structure then needs to mature and organize itself. That is why RF microneedling has two distinct effects, separated in time.

The first is immediate, from the contraction of existing collagen. It shows quickly, and it is also the more modest of the two. The second is delayed, from new collagen, and it is this one that gives lasting density to the skin. It unfolds gradually, over several months after the treatment.

Hence the logic of the course. The microneedling treatment is done 1–3 times with an interval of at least a month — the interval is there so the skin can work through the previous stimulus. A non-invasive course is built differently: 6–10 treatments every 7–10 days, then maintenance visits. Which schedule suits you is decided by your doctor individually — by the condition of your skin and the task, not by a universal timetable.

The good news is that the collagen you have built does not disappear on schedule. The result holds for a long time, and biorevitalization helps maintain it — it works on hydration and the skin’s reserves while RF is responsible for density. The question of a repeat course usually comes up no sooner than a year to eighteen months later, and your doctor decides it on the evidence, not by the calendar.

Who the method is not for

Device-based treatments look harmless, and that creates the false impression that they have no contraindications. They do, and some of them follow directly from the nature of radiofrequency energy — a pacemaker or metal implants in the treatment area, for example, require a separate discussion.

  • Oncological disease, including a history of skin cancer

  • Acute infectious and inflammatory processes, skin inflammation in the treatment area
  • A tendency to keloid scarring
  • Chronic conditions in a decompensated stage

How RF fits into a protocol

A treatment taken on its own almost always works less well than the same treatment in a considered sequence. RF lifting fits into a protocol by the calendar: biorevitalization is done 7–10 days before or after, mesotherapy alternates with the course, botulinum therapy is planned for 2–4 weeks afterwards. That way the methods reinforce one another instead of getting in the way of recovery.

The logic here is the same as in the clinic’s whole approach: the condition of the skin first, aesthetics second. RF gives density and structure, injectables give hydration and reserves, light-based methods give an even tone. Separately, each covers its own part of the task. Together, in the right order, they give the result a person came for in the first place.

Common questions

Does it hurt?

Before a microneedling treatment a topical anesthetic is applied, and the treated area is cooled with the CryoCool attachment. The reference point for the doctor is a sensation of warmth, not burning. Non-invasive fractional RF is comfortable and needs no anesthesia at all.

How soon will I see a result?

Part of the effect is visible almost at once: heated collagen fibers contract and the skin looks firmer. But the main part — the synthesis of new collagen — happens gradually and unfolds over several months after the treatment.

How many treatments are needed?

The microneedling treatment is done 1–3 times with an interval of at least a month. A non-invasive course runs to 6–10 treatments every 7–10 days, followed by maintenance. Your doctor sets the exact schedule individually, based on the task and the condition of your skin.

Will it leave marks on the skin?

After a microneedling treatment redness and swelling for 3–5 hours are possible, erythema for up to 1–2 days, sometimes fine crusts that clear within 3–7 days. This is the expected response to controlled injury, not a complication.

Is it suitable for darker skin?

Yes. Radiofrequency energy is not absorbed by melanin, so its safety does not depend on skin color. This is where RF has the advantage over light-based methods, which list darker skin among their contraindications.
